Note : You must register with DAVP to enter Fresh Empanelment details. After registration you will receive a User ID and Password on your E-mail ID. Using that User ID and Password, you can login and enter your Empanelment Details on-line.

After On-line Application submission, Please send hard copy of the application with documents with your ink signature and office seal.
